I am in-network with BCBS PPO (not Blue Choice). This means that I bill your insurance company and they pay a previously agreed upon fee for service, however you are responsible for any deductibles, co-pays, or co-insurance under the terms of your particular plan.

Please note that I do not confirm insurance coverage or benefits. I encourage all clients to contact their insurance company prior to scheduling to confirm coverage and any expected costs. This can be done by providing them with my NPI #1588295125.

I do not participate in other insurance plans. Patients pay for their treatment at the time of each visit unless other arrangements are made with the doctor. Many of my patients have insurance plans with generous out-of-network benefits that provide reimbursement for a substantial portion of my fees. I will supply the necessary documents for reimbursement filings.
